Information shared about your employer-sponsored group health coverage is provided each year to the IRS on Forms 1095-C, commonly referred to as Affordable Care Act (ACA) forms.

A copy of the return will be provided within 30 days of the date the request is received in the manner elected below.
Following separation of employment from Salina Public Schools, it is the responsibility of the individual to contact the company’s HR department to request a mailed or electronic copy of the information return form. After the close of the filing year, access to forms will no longer be available.
